We make final decisions on complaints about the NHS in England and UK government services. These casework decisions date from April 2021 and are published two months after the case is closed.
We changed our name to the Public Service Ombudsman on 1 October 2026. Decisions published before this date will be under our former name, Parliamentary and Health Service Ombudsman.
Published decisions reflect the legislation, caselaw and guidance in place at the time they were completed and depend on the individual circumstances of each case.
Please note: We do not publish the real names of individuals involved in a case.
